• Everyone entering the office will be required to fill the pre-screening COVID questionnaire. You will be contacted 24-72 hours prior to your appointment via phone, text or email and asked to fill out this form (available on our website ). It is required that this questionnaire be completed 24 hours (or less) before your appointment. We will have to reschedule your appointment if we are unable to complete this step.
 
  • Please postpone your appointment if you have been directed to isolate, quarantine, or feel unwell.
 
  • Please remember to bring a mask ( children included )
 
  • Your insurance plan is a contract between you and your insurance company. Please keep yourself up to date with your plan, maximums, cycle year, and benefits remaining BEFORE your appointment. We have no access to that information.
  • Please remain in your vehicle and call us on arrival. If the phones are busy or you do not have a cell phone, please ring the office doorbell to notify us you have arrived. We will advise you if the hallway is clear to ensure social distancing.

 

  • All patients are required to wear a mask in the office.

 

  • Only patients are allowed to enter the office, no companions unless a patient needs assistance or translation.

 

  • Maintain distancing in the reception area for essential caregivers and parents of minors if they cannot wait in a vehicle or outside the clinic.( Only one guardian allowed in with a minor patient, no parents allowed in the operatory during an aerosol procedure )

 

  • If not completed, we will ask you to complete the covid questionnaire outside, unfortunately this will use some of your appointment time, so please fill 24 hours or less prior to your appointment instead to avoid delays.

 

  • We will take your temperature using a contactless thermometer, and ask you to sanitize your hands. You will then be escorted to your treatment room.

 

  • Removed magazines and items that can harbor or transfer germs of any kind. Hand sanitizers will be positioned throughout the clinic.

 

  • Closure of the playroom.

 

  • Installed sneeze guards and barriers at the reception areas.

 

  • Closed up operator’s rooms with ambient air management with HEPA 13 air filtration continuously in treatment rooms and common areas to remove germs from circulating air. Air is fully exchanged in each room every 8 minutes.

 

  • UV sterilizers in each operatory for your personal items.

 

  • New protection equipment like visors, gowns, and masks for our doctors and team to provide barriers against the smallest of germs.
  • We will also ask you to please contact the office if you develop symptoms of COVID within 14 days after your appointment so we may notify the people who were working on the day of your appointment.
 
  • We have been forced to make the difficult decision of no longer accepting assignment.This means Fees are expected in full at the time of service. This will help us avoid charging a COVID fee or increasing our prices. Insurance forms will still be submitted on your behalf. If you lost your job or have been negatively affected by the pandemic please discuss it with us before your appointment so we can help your with possible arrangements.
