Your initial visit with us will include a comfortable, personalized, very thorough exam complete with Oral cancer screening, digital dental charting, x-rays and pictures of your teeth. Along with easy to understand information and choices about how we can help achieve your oral health goals.

 

Please plan to spend approximately 45-60 minutes with us for your dental exam.

 

Once we have a clear outlook of your oral health we will determine the hygiene treatment that is right for you. Hygiene treatment is uniquely customized for patients in our office. Our goal is to maintain and restore your teeth and gums in the most healthy, functional and comfortable manner possible. Your examination will include a specific hygiene plan based on the current condition of your teeth and gums. We will try our best to reserve time to co-ordinate your complete dental exam along with a full dental cleaning. Depending on the degree of difficulty of your professional dental cleaning, it may be required we see you again on a separate visit to complete your initial dental cleaning.

 

Please select the link ( under forms ) to complete your personal health history at home and submit the completed form online. Having this filled out in advance gives our team ample time to become familiar with your medical and dental history.

 

If you have other x-rays or other pertinent and recent records, please select the link to complete the ”Release Form” to request they be sent to us from your previous provider. We will send the form directly to your previous dentist on your behalf . This form is required in order to release and share your records with our office.

 

As a courtesy to you, we will be happy to familiarize ourselves with your dental plan benefits to determine an estimate of what your plan may contribute towards your treatment. If you wish to receive assistance, please be sure we have that information prior to your exam. Please bring your insurance card and booklet with you so we can keep a copy for your files. We provide detailed, electronic claim billing .

 

Payment is expected at the time of service. Credit card options are offered to make our treatment affordable and convenient.
